This video will assist all home care clinicians to obtain a medication history and perform a drug regimen review. While nurses and therapists (physical, speech and occupational therapy clinicians) are qualified(and expected/mandated) to perform the CMS-required Drug Regimen Review, all collaborating clinicians, working within their own scope of practice, must have knowledge about the process, and understand its impact on the patient's plan of care and outcomes. The video includes information on the relationship of the OASIS comprehensive assessment to the drug regimen review and the content of the drug regimen review as required by Medicare CoPs. The video also explores interdisciplinary collaboration and shows how to assess each drug's effectiveness, side effects and possible adverse effects. (40min.)
